Resolutions are due by March 2, 2025, for the 2025 Resolution Forum.
Resolutions will only be accepted through the form below. You will not be able to save this form and return to it later, nor make changes after it has been submitted.

Reminders Before You Submit
- Review and utilize the resolution writing tips to aid in writing your resolution.
- Be sure to use clear, concise language.
- Be as specific as possible about the desired action(s) you want the MAFP to take. (Reach out for assistance if you want or need help clarifying your goal/desired outcome.)
- Provide all information as it should be presented to members.
- If you used the resolution preparation worksheet, you may copy and paste the final content from the worksheet into the online submission form.
- Any formatting (like bold, italics, or superscripts) will be removed during submission.
