Minnesota Help Me Connect is an online service navigator that helps physicians/clinicians connect pregnant and parenting families with young children (birth-age 8) with services in their community that support healthy child development and family well-being. Help Me Connect’s mission is to connect families with resources that support the diverse needs of children and families living across the state of Minnesota, especially those experiencing racial, economic and geographic disparities.
Physicians/providers are able to connect patients with a variety of services, including the following:
- parenting programs on healthy development
- early childhood screening
- help with basic needs (food, housing, transportation)
- early intervention for special education, mental health and behavioral concerns
- free or low-cost legal services
- crisis supports
- early learning and child care programs
- community supports/services for disabilities
- affordable dental care
- tribal resources
- support for immigrants and refugees
- resources for families experiencing homelessness
- additional caregiving/community support/services
