Online Service Navigator for Patients With Young Kids

Minnesota Help Me Connect is an online service navigator that helps physicians/clinicians connect pregnant and parenting families with young children (birth-age 8) with services in their community that support healthy child development and family well-being. Help Me Connect’s mission is to connect families with resources that support the diverse needs of children and families living across the state of Minnesota, especially those experiencing racial, economic and geographic disparities.
