MAFP Academy Awards Nomination Form

The Family Medicine Educator of the Year award recognizes a member of the MAFP who is an outstanding teacher of family medicine

Who’s eligible: Full-time, part-time and volunteer physician educators from academic and clinical settings who are actively engaged in teaching (or were, within the past two years); must hold Active or Life membership in the MAFP to be considered

The Innovation & Research Award recognizes an individual who has made a major contribution to the development of family medicine research and/or innovation at the state and/or national level.

Who’s eligible: MAFP members, nonmembers, and non-physicians (all nominations must be made by MAFP members)

The Family Medicine Resident of the Year award recognizes a family medicine resident member of the MAFP for contributions to family medicine, including teaching, leadership and educational activities.

Who’s eligible: Resident members of the MAFP

The Medical Student Leadership Award recognizes a medical student member of the MAFP for family medicine involvement and leadership activities.

Who’s eligible: Student members of the MAFP
